Vaccinations
By signing your intials below, you agree to the following: All pets must be up to date on required vaccinations prior to their boarding stay. Parasite Prevention and Flea & Tick Preventative
By signing your intials below, you agree to the following: To prevent the spread of internal parasites, all pets are required to have proof from a veterinarian of a negative fecal. These records can be emailed dacusdoodles@gmail.com, or texted 936-697-9865 or 936-672-2091. To prevent the spread of internal and external parasites, all pets are required to be on a flea and tick preventative year-round. If your pet is found to have fleas or ticks while in our care, they will be treated at your expense.

If, at any time during your pet’s stay, a medical concern arises that does not require immediate attention, you will be contacted and advised of the situation, and we will follow your care instructions.

I certify that the information provided in this form is accurate to the best of my knowledge, and I have not withheld any information regarding our pet’s/pets’ health or behavior that could endanger our pet(s) or other pets at Dacus Doodles.

I understand that there are inherent risks to bringing my pet(s) to a Dacus Doodles to be boarded. I agree that Dacus Doodles,Tammy Seiter and Carli Lang will not be liable for any claims of injury, illness, damage, or death to my pet during its stay and that under no circumstances will Dacus Doodles, Tammy Seiter or Carli Lang be liable for consequential damages. I agree that I am responsible for any harm caused by my pet while in the care of Dacus Doodles. I shall indemnify Dacus Doodles, Tammy Seiter and Carli Lang against any claims made against it or for losses or damages suffered by Dacus Doodles as a result of my pet. I understand that I will be responsible for any and all costs incurred during this and any future boarding stays.
